Integrative Therapeutic Paradigms for Autism Spectrum Disorder: Analyzing the Synergistic Relationship Between Nutritional Biochemistry and Physical Rehabilitation

ABSTRACT
Background: Autism Spectrum Disorder (ASD) is a systemic neurodevelopmental condition frequently accompanied by physical challenges, including gastrointestinal distress, hypotonia, and motor deficits. Traditional care models often treat these metabolic and musculoskeletal symptoms in isolation, which may limit overall therapeutic efficacy and place additional burdens on caregivers.

Objective: This study aimed to explore the lived experiences and clinical perceptions of caregivers and healthcare professionals utilizing a synchronized care model that integrates nutritional management with physical rehabilitation.

Methods: Following a qualitative descriptive design aligned with COREQ guidelines, 28 participants (18 caregivers and 10 healthcare professionals) were interviewed at the Dhaka College of Physiotherapy. Data were collected via semi-structured in-depth interviews and focus group discussions, and analyzed using Braun and Clarke’s thematic analysis framework with multiple coders to ensure trustworthiness.

Results: Three primary themes emerged: (1) Metabolic Efficiency, where participants perceived that addressing nutritional deficits provided the baseline stamina needed for therapy; (2) Structural Support, highlighting the perceived bidirectional need for nutritional building blocks and mechanical weightbearing exercises; and (3) Neuro-Efficiency, revealing that mitigating gastrointestinal distress was clinically observed to reduce sensory reactivity and support motor learning.

Conclusion: The findings suggest that integrated nutritional management and physiotherapy may improve coordinated care and warrant further evaluation in larger mixed-methods and intervention studies
